Terms of Use

Last updated: October 7, 2026

Welcome to amareics.com, the website of Amare Integrated Clinical Services, LLC (“Amare ICS,” “we,” “us”). By using this website, you agree to these Terms of Use. If you do not agree, please do not use the website.

Information only, not treatment

The content on this website is for general information and education. It is not therapy, diagnosis or medical advice, and it does not replace care from a qualified professional who knows your situation.

No therapist-client relationship through the website

Visiting this website, sending a message through the contact form, or calling or emailing us does not create a therapist-client relationship. That relationship begins only after you complete intake and sign our Consent for Services.

Not for emergencies

Amare ICS does not provide emergency or after-hours crisis services, and this website and our contact channels are not monitored for emergencies. If you are in crisis or in danger:

  • Call or text 988 (Suicide & Crisis Lifeline, available 24/7 in English and Spanish)
  • Call 911
  • Go to the nearest emergency room

Who we serve

Amare ICS serves adults 18 and older and adolescents ages 12 to 17. Clinical services are provided by telehealth only, to people who are physically located in Arizona at the time of each session. Services for anyone under 18 require written consent from a parent or legal guardian.

Consulting services

Amare ICS also offers behavioral health consulting to organizations, provided under a separate written consulting agreement. Consulting services are advisory only and do not include direct clinical services to any organization's residents or clients.

Supervised private practice

Amare ICS is a supervised private practice. It is owned and managed by a master's level, non-independent licensee under Board-approved clinical supervision pursuant to A.A.C. R4-6-217. The Board-approved clinical supervisor of this practice is Ryan Olson, LCSW.

Communications

The contact form, regular email and text messages are not secure channels for health information. Please share only your name, contact details and a general reason for reaching out. Once you become a client, our communication moves to our secure client portal.

Fees and policies

Amare ICS is a private-pay practice and does not bill insurance or AHCCCS. Fees, payment, cancellation and other service terms are explained in our Financial Agreement and in your Good Faith Estimate, both provided before your first session. Fees shown on this website may change; the fees in your signed agreement are the ones that apply to you.

Your right to a Good Faith Estimate

Under the federal No Surprises Act, you have the right to receive a written Good Faith Estimate of the expected cost of your care before services begin. Health care providers must give this estimate to people who don't have insurance or who aren't using insurance. Because Amare ICS doesn't bill insurance, every client receives one.

  • When you'll receive it: within 1 business day of scheduling, if your first appointment is at least 3 business days away, or within 3 business days of scheduling, if it is at least 10 business days away. You can also ask for an estimate at any time, even before scheduling, and receive it within 3 business days.
  • What it covers: the expected cost of the services you plan to receive, such as individual, couples, family or adolescent therapy, or support groups, for up to 12 months, at the fee you will actually pay, including Income-Based Program fees if you are enrolled. It covers Amare ICS services only. It is an honest estimate, not a contract or a guarantee, and your actual costs may change with your needs and attendance. If your services, fees or session frequency change significantly, or after 12 months, you will receive an updated estimate.
  • If your bill is much higher: if your bill from Amare ICS is at least $400 more than your Good Faith Estimate, you can dispute the bill through the federal patient-provider dispute resolution process. You have 120 days from the day you receive the bill to start, and there is a $25 administrative fee. Filing a dispute will not affect your care at Amare ICS in any way. Keep a copy or a picture of your Good Faith Estimate.

Questions about your estimate? Contact us at info@amareics.com or (928) 940-2120. For more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.

Amare ICS does not discriminate based on race, color, national origin, ethnicity, immigration status, sex, gender identity or expression, sexual orientation, age, disability, religion, marital status, pregnancy, socioeconomic status or source of payment. Services are available in English and Spanish, and disability accommodations are provided at no cost.
